Third Grade Requirements

• Third graders needs to provide a certificate of vision screening.

Seventh Grade Requirements

• Students entering 7th grade are required to have a one-time dose of tetanus, diphtheria, acellular pertussis (Tdap) vaccine and one dose of meningococcal conjugate vaccine (A,C,W,Y).

Ninth Grade Requirements

• Students in 9th grade need to provide a certificate of dental screening.

Twelfth Grade Requirements

• Students entering 12th grade are required to have two doses of meningococcal conjugate vaccine (A,C,W,Y); or 1 dose if one dose was previously received when the student was sixteen years of age or older.
